Access Affordable Care Services at Home

Government-funded Home Care Packages help older Australians access affordable care services at home. These packages are delivered under the Consumer Directed Care (CDC) model, giving you flexibility and control over the types of services received and how they are delivered.

Frequently Asked Questions

An Aged Care Assessment will be undertaken that will outline your care needs and services available to support you at home.

There are four levels of Home Care Packages for different care and support needs:

Level 1 for basic care needs

Level 2 for low-level care needs

Level 3 for intermediate care needs

Level 4 for high-level care needs.

Each level receives a different amount of government funding. The higher the level, the more funding the government pays, allowing access to more support, services or equipment.

At St Anna’s your home care package will be adapted as your needs change. Your Home Care Coordinator and clinical staff will work with you to ensure that your care plan is right for you and assist with applying for reviews through My Aged Care if your care needs increase or change.

St Anna’s offers a range of fee for service supports for people who are either awaiting a Home Care Package, receiving CHSP (Commonwealth Home Support Program) or are ineligible for either but need some assistance at home. Contact St. Anna’s to discuss your needs and options.

Contact My Aged Care and let them know that you are changing providers, you will be given a referral code to provide us. You will need to agree on an end date with your current provider, it’s a good idea to ensure you are familiar with any exit fees or conditions in your agreement. Contact our friendly staff to discuss your care needs and we will be able to guide you through the process.

At St. Anna’s, we offer a wide range of care options to assist in supporting you to stay in your own home and keep doing the things that you love. This includes but is not limited to;

Each Home Care Package level is funded at a different daily rate depending on your level of care needs. You may also be eligible for additional supplements subject to certain criteria, your Home Care Coordinator will assess these options and assist you to apply for them. Home Care Package fees may also include;

A basic daily fee – this is an amount you can self-contribute to your package to increase the funds available to you in your Home Care Package budget.

Income-tested care fee – an extra contribution that some people pay, as determined through an income assessment. It’s based on individual income including your pension. However, full pensioners do not pay an income-tested care fee.
