Are you looking for some extra support to assist you to live independently and maintain your health and wellbeing within the comfort of your own home?

Here at St. Anna’s, we pride ourselves on being a service with a heart.
St. Anna’s provide tailored home care services through our extensively trained team, who support our clients to live to their full potential.  Our Home Care Coordinator, together with yourself, carers, families and friends work to create an individualized home care package to suit you preferences, required to maintain a healthy and active lifestyle.  Services are always negotiable and tailored to your needs.

We understand that things change, therefore our Home Care Coordinator works hard to ensure that the individual needs of each client are met by consulting with the client, client family members, Aged Care Assessment Team (ACAT) and Allied Health members, to ensure our clients are maintaining their health and wellbeing.

Our Home Care service operates within the Adelaide Metropolitan boundary and is established to assist older persons within the community to remain at home as long as possible.

People experiencing financial hardship are not excluded from our program and each client’s situation is assessed individually.

To qualify for a Home Care Package, you are required to have an assessment completed by the Aged Care Assessment Team (ACAT), who once organized, will visit you in your home for assessment.  Your local doctor or relative can assist you with this.
